Intake and Assessment $160

Individual Therapy $140 per hour

EMDR Extended Sessions $140/hr with 50% deposit

Insurance & Payment

I am in network with the insurance companies listed below. However, EMDR extended sessions are not covered by insurance companies, only individual weekly sessions lasting up to 60 minutes. Payment plans may be made available for the the remaining balance after deposit. I accept cash, check, or credit card at the time of service. A superbill, or monthly statement including provider and service information, can be provided for clients out-of-network to submit to their insurance for personal reimbursement upon request. Please be aware a diagnosis is necessary for insurance reimbursement. Please connect with me should you have any questions regarding insurance and payment.

Cancellation Policy

I require 24 hours notice of cancellation. If you do not appear for your session without notifying me you will be required to pay the full cost of the session. If notified less than 24 hours prior to the appointment time, a $100 late cancellation fee will be applied (emergencies and illness do not apply). EMDR extended session deposits are nonrefundable.
